What is Pediatric Case Management?

When a child has complex medical needs, families often juggle specialists, therapists, equipment companies, insurance and school. A case manager pulls it all together. They track your child’s care plan, talk with providers for you, and help you stay ahead of changes.

What Our Case Managers Will Do

Our case managers will help help with tasks including, but not limited to:

  • Updating your child’s care plan as their needs change
  • Communicating with pediatricians, specialists and therapists
  • Scheduling and coordinating nursing and aide visits
  • Supporting your family through transitions, like hospital discharge or a change in services
  • Helping with insurance requirements, authorizations and renewals
  • Connecting you with resources like your regional center and California Children’s Services (CCS)

Who Is Pediatric Case Management For?

Case management is for families who are managing a lot of moving pieces in their child’s care. It’s often a good fit for:

  • Children who see several specialists and need everyone on the same page
  • Children recently home from the hospital or NICU with new orders, equipment and follow-up visits
  • Families working with a regional center, CCS or school services alongside medical care
  • Families juggling insurance authorizations and renewals for nursing hours or supplies
  • Parents who feel overwhelmed by appointments, calls and paperwork
